Enhancing the quality of care for patients in all care settings through partnership collaboration, advice and education

Oakhaven Hospice Trust provides specialist palliative care and support to those facing life-limiting illness, and their loved ones, in both a home and hospice setting. Funding has recently been secured from the Strategic Health Authority giving us the opportunity of providing further support, free of charge, to local care homes by working alongside their staff and to help develop and enhance the skills in caring for those nearing the end of life.
We do this by liaising closely with care home staff in the following ways;
- By discussion and identifying any palliative care needs that may be needed and formulating a supportive education/training programme accordingly;
- Setting up small group discussion sessions based on cases that have been experienced;
- Oakhaven nursing staff working alongside the care home staff when caring for the dying;
- Care home staff working alongside Oakhaven palliative care staff within the Hospice environment.
Our aim is to help in increasing skills and confidence in practitioners who are involved with patients and families at the end of life.
This project is currently being expanded with more developments in the near future, and we will update this page when we have any news.
