Oakhaven Hospice Clinical Team

Oakhaven Hospice is a specialist palliative care service providing care to patients in the New Forest and Waterside.

Oakhaven Hospice provides specialist care to patients, carers and families who are facing complex physical, emotional and practical difficulties arising from advanced progressive life limiting illness.  This may be cancer or other diseases.  We believe in respecting the individual's dignity and choices.  We enhance the quality of care for patients in all care settings and through partnership and collaboration, advice and education.

Your referral to the Oakhaven specialist palliative care team enables you to access the skills and experience of a wide range of professionals. Members of the team work together to achieve the best possible care for patients and their families both during their illness and during  bereavement. One of the team will be allocated as your key worker who will be your first point of contact.

Meet the Clinical Team:

Nursing
Nurses with different skills and experience work across all settings.  Community Clinical Nurse Specialists provide advice and support to patients and their families in their own homes. They are supported by the Oakhaven at home nursing team who provide hands on care to enable people to stay at home if that is their wish.  They meet regularly with GPs and district nurses and work closely with other care agencies.   Nurses in the in-patient unit provide hands on personal care and support patients and their families during their stay.  In the Day Hospice nurses support and care for patients who attend for the day on a weekly basis.

Medical
There are 2 consultants in palliative medicine supported by other doctors with specialist skills and experience who care for patients in the in-patient unit, in the community and the day hospice.  

Physiotherapy
We have a senior physiotherapist and physiotherapy technician offering help and advice on symptom management and ways to remain as independent as possible.  They work in the the in-patient unit, in the community and the day hospice.  

Complementary therapy
A qualified complementary therapy co-ordinator leads a team of qualified volunteer complementary therapists.  They work in the in-patient unit, in the community and the day hospice. Treatments offered are reflexology, aromatherapy, massage and acupuncture for patients and their families.  complementary therapy can help with symptom management and provide relaxation.

Counselling
We have two qualified counsellors and a team of volunteers who can help discuss any personal difficulties  which you , your family or friends may be experiencing whilst coping with your illness.  Our counselling service also offers bereavement care.  The team works in the in-patient unit, in the community and the day hospice.  

Chaplaincy
Chaplains provide religious and spiritual care to patients and their families.  They work in the in-patient unit, the community,  the day hospice and in bereavement care.  

Pharmacy
A pharmacist from Lymington hospital visits twice a week and supports staff in the use of medicines.

Occupational Therapy
We have limited access to an occupational therapist for the in patient unit.  Occupational therapists advise on adaptations in the home and access the necessary equipment.

After your initial assessment a care plan will be agreed with you and your family.  Your key worker will regularly review your case with the full multi-professional team and discuss any complex issues that arise.
