Welcome to Oakhaven Hospice at Home we provide hands-on specialist palliative care for people who wish to be at home.
We established the service in 2008 and we now have a specialist team of nurses working alongside GP’s, Community Clinical Nurse Specialists, District Nurses and other agencies.

Our nurses whilst assessing the patient provide symptom control and care in their home at the end of life. They also support the families by offering practical advice where appropriate and help them to come to terms with the impact of bereavement. We are always very pleased to share with others our specialist knowledge.

Training and support for the team is vital to enable them to keep abreast of new developments and advancements in end of life care. Taking the hospice into the home brings with it its own problems. For example incorporating specialist equipment such as hoists, hospital beds etc which enables us to best manage the patient’s needs. In addition to mandatory training, Oakhaven at Home has bi monthly team meetings and training sessions where a wide variety of topics are discussed. It is also an opportunity for the nursing team to come together to exchange views and ideas.
We have discovered over the years that families are hugely appreciative of the care we can offer and there is an overwhelming need for our service, particularly for night care where there are huge deficiencies when trying to care for the terminally ill at home and avoid unnecessary admissions to hospital.

In addition to our normal activity we are also actively involved with The National Association of Hospice at Home.
