In hospital

Lymington Hospital

We work closely with our NHS colleagues in all settings. If you are in hospital the clinicians caring for you may feel that you can best be cared for by Oakhaven. If this is the case they will speak with you and you may be discharged straight from hospital to our hospice.

Our hospice staff also visit hospitals to support our NHS colleagues and to help meet your needs. In particular this is with Lymington New Forest Hospital but Oakhaven consultants also regularly attend and support Southampton General to help with your care and support your discharge either back to your home or to our hospice.

Lymington New Forest Hospital

If you are in Lymington New Forest hospital and are already known to Oakhaven one of our consultants or nurse specialists can see you there and liaise with the hospital staff about your treatment, care and any discharge plans. This may include transfer over to Oakhaven if you would like and where it is appropriate.

If they think it would be helpful for you, the hospital staff may refer you to us, Oakhaven may advise the team about controlling your symptoms, provide emotional support and help with any discharge plans to home, care home or transfer to Oakhaven.

We all provide regular education sessions for the hospital staff and work closely with them on palliative care issues.
