  • Supporting Information

  • This part of the application form gives you an opportunity to provide further information in support of your application. Please demonstrate how previous and present experience enables you to meet the job description and satisfy the essential and desirable criteria on the person specification and provide details of any other skills you have that are relevant to the role to which you are applying. You may wish to continue on a separate sheet if necessary. A decision to shortlist can only be based on the information provided
  • Criminal Records Disclosure

Only relevant convictions and other information will be taken into account in the recruitment and selection process so disclosure need not necessarily be a bar to obtaining this position.

If you declare a criminal record and we believe this to have a bearing on the requirements of the post, we will discuss the matter with you at interview. If we do not raise the record with you it is because we have taken the view that it should not be taken into account in deciding your suitability for the post. If you require further information or have any concerns about filling in this disclosure, please contact the Human Resources Manager on 01590 613030
  • There are a number of posts and professions that are exempt from the provision of the Rehabilitation of Offenders Act 1974. These include posts where, in the normal course of their duties, successful applicants will have access to vulnerable adults.

If the post you have applied for falls within the above category, you will be required to apply for a Disclosure & Barring Service check. Applicants will not be entitled to withhold any information about convictions which for other purposes are ‘spent’ under the provisions of the Act and in the event of employment; any failure to disclose such convictions could result in dismissal or disciplinary action by Oakhaven Hospice. Any information given will be confidential and will be considered only in relation to posts to which the order applies.

The job description will state if the role requires us to carry out a DBS check.

  • Equal Opportunities Monitoring Form

The following page will be detached from the rest of your application form and will not be used in the selection process, but may be used for monitoring purposes. The form will be part of your personnel record should you be appointed.
  • Equal Opportunities

Oakhaven Hospice has an Equal Opportunities policy, the aim of which is to ensure that no job applicant or employee receives less favourable treatment or is disadvantaged by requirements which cannot be shown to be justified. In order to ensure that this policy remains effective, you are requested to answer the following questions (but it is not a legal requirement). This form will be treated in the strictest confidence and is designed for monitoring purposes only.
  • MM slash DD slash YYYY
  • Disability

The Equality Act 2010 defines disability as ‘a physical or mental impairment which has a substantial long-term adverse effect on a person’s ability to carry out normal day-to-day activities’.

  • Exemption from the Rehabilitation of Offenders Act 1974

Convictions will not necessarily be a bar to obtaining a post within the Hospice. However because of the nature of the work for which you are applying, this post is exempt from the provisions of Section 4(2) of the Rehabilitation of Offenders Act, 1974. Applicants must declare information about convictions, cautions, reprimands and final warnings, which for other purposes are “spent” under the provisions of the Act. In the event of employment any failure to disclose such convictions could result in disciplinary action or dismissal by Oakhaven Hospice Trust.
